Customer retention forms
All storage devices removed from customer machines will be left with the customer. To acknowledge receipt, the customer will sign for the device on the included customer retention form (or you may use one of the alternate language versions found below).
Write in debrief the name of customer who signed the document and send copy to naspoc@lexmark.com.
